Background There is considerable variation in the functionality of bioethical services in different institutions and countries for children in hospital, despite new challenges due to increasing technology supports for children with serious illness and medical complexity. We aimed to understand how bioethics services address bioethical concerns that are increasingly encountered in paediatric intensive care. Methods A qualitative descriptive design was used to describe clinician’s perspectives on the functionality of clinical bioethics services for paediatric intensive care units. Clinicians who were members (...) of formal or informal clinical bioethics groups, or who were closely involved with the process of working through ethically challenging decisions, were interviewed. Interviews took place online. Resulting transcripts were analysed using thematic analysis. Results From 33 interviews, we identified four themes that described the functionality of bioethics services when a child requires technology to sustain life: striving for consensus; the importance of guidelines; a structure that facilitates a time-sensitive and relevant response; and strong leadership and teamwork. Conclusions Clinical bioethics services have the potential to expand their role due to the challenges brought by advancing medical technology and the increasing options it brings for treatment. Further work is needed to identify where and how bioethics services can evolve and adapt to fully address the needs of the decision-makers in PICU. (shrink)

The present worldwide ecological crisis challenges both some fundamental Western cultural assumptions about human relationships to nature and the efficacy of democratic institutions in transforming these relationships appropriately and in a timely manner. I discuss what kind of ecophilosophy is most feasible and desirable in guiding rapid and effective response to the present crisis in the short term, as well as positive cultural transformation in the West toward sound natural and social ecology in the longer term. I argue that decontextualized (...) liberal ecophilosophies and related deep ecologies are inadequate to these purposes and propose a Green transformative framework that “re-places” humans within nature, “re-positions” our understanding of ourselves in relation to the land, “re-pairs” intrinsic values in nature with human responsibilities, and “re-directs” the effective use of participatory democratic institutions in transforming public policy. Diversity is both an unavoidable aspect of twenty-first century living and a powerful challenge to older philosophical traditions that still assume as normatively universal a set of values, ways of thinking, institutions, and habits of living that emerged within earlier eras of more homogeneous cultures, less developed technologies, and more accepted forms of linguistic, legal, religious, economic, political, and military domination. Within recent years, new styles of philosophical discourse, including deconstruction, postmodernism, feminism, post-colonialism, and critical race theory, have persuasively challenged (...) these universalistic assumptions to reveal the important human differences they marginalize. Experience-based appreciation of the mutually educative potential of diverse standpoints as well as sober concern about the perils of our present times have led many thinkers to look for contemporary forms of pragmatism and cosmopolitanism as hospitable intellectual gathering places for urgently needed cross-difference conversations that may reflect and give substance to shared visions of democratic diversity. The eight authors in this volume engage in cross-difference conversations with other thinkers from earlier periods and other philosophical traditions, as well as with each other, in order to reconstruct pragmatism and cosmopolitanism in ways that are more attuned to our lived experience of diversity as well as our hopes for a diversity-appreciating democratic future. The purpose of this workshop is to share our current work-in-progress and solicit feedback and ideas from our colleagues as we begin to design a research study based on a paper we presented at the 2005 Academy of Management conference, “The Ethical Implications of Power in Organizations.” Our paper examines the nexus of power and ethics in organizations, and how they are treated in the management, sociology, and psychology literature. Our discussion assumes a wide range of uses and abuses of (...) power, including but not limited to sexual harassment, anti-labor practices, excessive executive compensation, manipulation of stock prices, discrimination, environmental degradation, etc. In addition we surface and discuss the assumptions, norms, paradoxes, and practices of power in organizations in relation to business ethics. Since 9/11, citizens of all nations have been searching for a democratic public philosophy that provides practical and inspiring answers to the problems of the twenty-first century. Drawing on the wisdom of past and present pragmatist thinkers, Judith M. Green maps a contemporary form of citizenship that emphasizes participation and cooperation and reclaims the critical role of social movements and nongovernmental organizations. Starting with empowering processes of storytelling, truth and reconciliation, and collaborative vision-questing that allow individuals to give (...) voice and new meaning to their loss, anxiety, and hope, Green frames cooperative inquiries to guide transformative actions. From this "second strand" of the democratic experience, leaders and participating citizens can help to shape a more desirable democratic future. In dialogue with Richard Rorty, Judith Butler, James Baldwin, Martin Luther King Jr., Elie Wiesel, Viktor Frankl, Cornel West, and other contemporary thinkers, Green defines the need for deeper understanding and fulfillment of the potentials of the democratic ideal. Drawing insights from Thomas Jefferson, Walt Whitman, William James, John Dewey, Jane Adams, and other earlier thinkers, Green frames a pragmatist understanding of emerging realities and possibilities, growing wells of shared truths, multifaceted histories, and mutually transformative experiences of citizenship. Employing examples from America's complex history and from recent world events, Green locates four sites for effective citizen activism: government at all levels, nonprofit organizations, issue-focused campaigns and social movements, and daily urban living. Green shows how citizens can revive social hope and deepen the democratic experience by drawing on their own knowledge and developing their capabilities through inclusive civic participation. Clinical experience suggests that adult survivors of childhood trauma arrive at their memories in a number of ways, with varying degrees of associated distress and uncertainty and, in some cases, after memory lapses of varying duration and extent. Among those patients who enter psychotherapy as a result of early abuse, three general patterns of traumatic recall are identified: relatively continuous and complete recall of childhood abuse experiences coupled with changing interpretations of these experiences, partial amnesia for abuse events, accompanied by (...) a mixture of delayed recall and delayed understanding, and delayed recall following a period of profound and pervasive amnesia. These patterns are represented by three composite clinical vignettes. Variations among them suggest that the phenomena underlying traumatic recall are continuous not dichotomous. Future research into the nature of traumatic memory should be informed by clinical observation. (shrink)

Schizophrenia is a severe mental illness, which affects sense of identity. While the ability to have a coherent vision of the self relies partly on its reciprocal relationships with autobiographical memories, little is known about how memories ground “self-images” in schizophrenia. Twenty-five patients with schizophrenia and 25 controls were asked to give six autobiographical memories related to four self-statements they considered essential for defining their identity. Results showed that patients’ self-images were more passive than those of controls. Autobiographical memories underlying (...) self-images were less thematically linked to these self-images in patients. We also found evidence of a weakened sense of self and a deficient organization of autobiographical memories grounding the self in schizophrenia. These abnormalities may account for the poor cohesiveness of the self in schizophrenia. (shrink)

Nursing Inquiry 2010; 17: 297–308 The power and politics of collaboration in nurse practitioner role developmentThis health services study employed participatory action research to engage nurse practitioners (NPs) from two health authorities in British Columbia, Canada, to examine the research question: How does collaboration advance NP role integration within primary health‐care? The inquiry was significant and timely because the NP role was recently introduced into the province, supported by passage of legislation and regulation and introduction (...) of graduate education programs. In separate and concurrent inquiry groups, the NPs discussed their practice patterns, role development progress and understanding of collaboration and role integration. The inquiry revealed the political nature of the NP role and the extent to which NPs relied on collaborative relations at all levels of the health system to advance role integration. Given that NP role development is still at an early stage in this province, as well as other provinces in Canada, this study provides important insights into the power and politics of role development, and offers direction for future role advancement. Since 1990 I have been deeply involved in the development of Latin American ecofeminist thought and its theological, ethical and spiritual perspectives as a founding member of the Con-spirando Collective, a team of women working in the areas of ecofeminist theology, ethics and spirituality in Santiago, Chile. This article describes the results of a research project I conducted based on interviews with twelve faith-based activist women who had historically aligned themselves with liberation theology and its practice and who now describe (...) themselves as ecofeminists. The aim was to document the shift that took place in their identities and their growing ecofeminist awareness. This is made visible through the ways that these women perceive themselves in relation to the rest of the Earth community and to the Universe as a whole; in the way they re-image/re-name Ultimate Mystery; in their beliefs about death and rebirth; and in their spiritual and ethical practice. Feminist critics have charged that Aristotle's mistaken and harmful remarks about women and slaves show inconsistency or bias-driven arbitrariness. However, this analysis shows that these remarks function within a consistent and coherent theoretical corpus. Thus, both Aristotle's hierarchical and dualistic first principles and the methodology on which his entire corpus is based must be unreliable. Moreover, consistency and coherence must be insufficient warrants of theoretical insightfulness. Aristotle's mistakes suggest caveats for feminist philosophical reconstruction.

The passing of Richard Rorty is an event to mark in the annals of American philosophy - the passing of a spirit-guide to some, and of a dark shadow to others, but certainly that of an original, iconoclastic thinker who brought classical American pragmatism back into the contemporary philosophical conversation, and who got philosophers telling stories of achieving a long-loved dream of democracy. There has been a tendency, natural perhaps in such ‘verbal’ disciplines as philosophy and linguistics, to assume that language and communication are the same thing. But while no one would deny that language is one powerful medium of human communication, is it the only one? Is there any real distinction between communicating one's desire to leave a dinner party by making verbal remarks like, ‘I must go’ or ‘We could only get Jane as a babysitter’, as opposed to fidgeting, standing (...) up and looking longingly towards the door? As Michael Argyle argues, a great deal of information is conveyed by non-verbal cues. To take the argument a step further, are even verbal statements examples of purely linguistic communication? Whereas the manifest linguistic content of ‘I must go’ is obvious to anyone who speaks English, the meaning of the remark about the babysitter can only be understood by those who know the particular neighbourhood social context in which it was uttered. Does it make sense, then, to try and analyse the linguistic structure and content of an utterance without taking into account the use to which it is being put in a particular extra-linguistic context? This article focuses on the relationship between Barack Obama and pragmatism by reframing Barack Obama's deeply held values, carefully considered intellectual commitments, highly developed gifts, hypothetically framed transformative strategies, and their emerging outcomes in light of works by classical and contemporary philosophical pragmatists in order to help us clarify how we can advance pragmatism's meaning and relevance in the twenty-first century.

The rural health context in the United States presents unique ethical challenges to its approximately 60 million residents, who represent about one quarter of the overall population and are distributed over three-quarters of the country’s land mass. The rural context is not only identified by the small population density and distance to an urban setting but also by a combination of social, religious, geographical, and cultural factors.
